The mailer (envelope) served as sleeve and was sent by Halo  Shampoo and 
states:
Here's Your Special Collector's Iem Record from  Halo!



I got this disc (as a 45) when it first came out.  Way back when....  IAnd 
another one, IIRC, but will have to check.)
