But is sleeve is almost identical to NBC's, down to the layout and warning
messages.

Here's one from my collection:

I just noticed that the NBC sleeve uses the term "Orthacoustic," while the
Australian sleeve adds an "O" and renders it "OrthOacoustic." Did licensing
of the curve entitle a broadcaster to use the template for the sleeve
design?

This 16" Australian ET ended on eBay recently. The sleeve is most
interesting; almost identical to NBC Orthacoustic ET sleeves of the
period,
down to the notice in the upper right corner:

Did Amalgamated Wireless have a relationship with NBC?

Orthoacoustic was an RCA equalization curve, and was not exclusively
NBC. Therefore there was not necessarily any connection between AWA and
NBC, just that AWA used the RCA curve. The disc in that sleeve, of
course, has nothing to do with the sleeve.
