I didn't realize you were talking about constant linear velocity 
records. The Sound Forge tool you want to use for this is "Pitch 
Bend", not "Pitch Shift." You can find this under the Effects menu.

The Pitch Bend tool allows you to draw an envelope describing how to 
change the pitch over time. For a record that starts at 33 1/3 RPM 
and ends at 78.26 RPM, first digitize the whole thing at 33 1/3. Then 
open the Pitch Bend tool and click the Reset Envelope button. Next, 
move the Range slider to the maximum (24 semitones). Then "grab" the 
"handle" on the right-hand end of the envelope and slide it up to 
14.8, which is the number of semi-tones between 33 1/3 and 78.26. 
Make sure that "Preserve Original Duration" is *NOT* selected. Then 
click "OK" - this will cause Sound Forge to increase the speed 
linearly by 14.8 semitones over the range of the recording.

I don't know whether the initial speeds and final speeds of 33 1/3 
and 78.26 are correct. I assume you can figure this out and determine 
how to change the tool settings accordingly.
