Sorry, but are you sure the old lady was watching you ? No doors ? The 
chain was supposed to flush the toilet, not to hold yourself up !

I"ve been in public places in  NYC where the door was only half-height, 
and there was a guy sitting on a stool watching the scene. "Museum of 
Natural History", 1968 ...

Now about the 'urinals': it's possible that you're talking about 
'robinets' (water dispensers) which were necessary because there was no 
running water in your room ??

So, your stories are very funny to me (kind of Borat in reverse)
